Almost all products of these genes are expressed in the structures of the neuromuscular system, which makes it possible to classify distal arthrogryposis as a neuromuscular disease. Type 7 distal arthrogryposis is a rare autosomal dominant disease characterized by two main symptoms: mandibular trismus and pseudocamptodactyly, a specific symptom of limited mobility of the interphalangeal joints during hand dorsiflexion with no restriction during palmar flexion. In all patients described in the literature from different populations with type 7 distal arthrogryposis, the same pathogenic variant c.2021G&gt;A(p.Arg674Gln) was found in the <italic>MYH8</italic> gene, the protein product of which is one of the myosin isoforms functioning in the embryonic period and providing the formation of muscle fiber structures.</p><p>The aim of the work is to describe the clinical and genetic characteristics of the first family case of type 7 distal arthrogryposis in Russian patients. The patients underwent clinical examination and electromyography. Exome sequencing after DNA isolation from the proband’s blood according to the standard method was carried out on the NextSeq 500 platform (Illumina, USA) using the pairedend reading method (2 × 75 bp). Confirmation of the pathogenicity of the identified variants was carried out using automatic Sanger sequencing.</p><p>As a result of molecular genetic analysis in a father and son with clinical manifestations of type 7 distal arthrogryposis, a heterozygous c.2021G&gt;A variant in exon 18 of the <italic>MYH8</italic> gene, which was previously described in all patients published in the literature, was detected, leading to the replacement of p.Arg674Gln(NM_002472.2) in a protein molecule. The examined patients did not reveal focal neurological symptoms, as well as minor developmental abnomalities, pathology of internal organs, ulnar deviations, equinovarus feet deformities, vertical orientation of the talus, contractures of the hip joints, which were found with varying frequency in previously described patients with variants in the <italic>MYH8</italic> gene. Specific clinical signs of type 7 distal arthrogryposis, combined with the presence of a major nucleotide variant, make it possible to optimize the process of molecular genetic diagnosis of this type of hereditary arthrogryposis.</p></abstract><trans-abstract xml:lang="ru"><p>Дистальные артрогрипозы – группа генетически гетерогенных врожденных болезней, характеризующихся непрогрессирующими контрактурами преимущественно дистальных суставов верхних и нижних конечностей. Идентифицировано 11 генов, патогенные варианты в которых обусловливают возникновение аутосомно-доминантных и аутосомно-рецессивных типов дистальных артрогрипозов. Практически все продукты этих генов экспрессируются в структурах нейромоторного аппарата, что позволяет отнести дистальный артрогрипоз к нервно-мышечным болезням. Дистальный артрогрипоз 7-го типа – редкое аутосомно-доминантное заболевание, характеризующееся 2 основными симптомами: тризмом нижней челюсти и псевдокамптодактилией, специфическим симптомом в виде ограничения подвижности межфаланговых суставов при тыльном сгибании кисти и отсутствия ограничения при ладонном сгибании. У всех описанных в литературе пациентов из разных популяций с дистальным артрогрипозом 7-го типа обнаружен один и тот же патогенный вариант с.2021G&gt;A(p.Arg674Gln) в гене <italic>MYH8</italic>, белковый продукт которого является одной из изоформ миозина, функционирующего в эмбриональном периоде и обеспечивающего формирование структур мышечных волокон.</p><p>Цель настоящей работы – описание клинико-генетических характеристик первого семейного случая дистального артрогрипоза 7-го типа у российских пациентов.</p><p>Пациентам были проведены клинический осмотр и электромиография. Секвенирование экзома после выделения ДНК из крови пробанда по стандартной методике выполняли на платформе NextSeq 500 (Illumina, США) методом парно-концевого чтения (2 × 75 п.о.). Подтверждение патогенности выявленных вариантов проводили с помощью автоматического секвенирования по Сэнгеру.</p><p>В результате молекулярно-генетического анализа у отца и сына с клиническими проявлениями дистального артрогрипоза 7-го типа выявлен описанный ранее у всех опубликованных в литературе пациентов гетерозиготный вариант c.2021G&gt;A в экзоне 18 гена <italic>MYH8</italic>, приводящий к замене p.Arg674Gln(NM_002472.2) в белковой молекуле. У обследованных пациентов не выявлено очаговой неврологической симптоматики, а также малых аномалий развития, патологии внутренних органов, ульнарной девиации кистей, эквиноварусной деформации стоп, вертикальной ориентации таранной кости, контрактур тазобедренных суставов, которые с различной частотой обнаруживались у ранее описанных пациентов с вариантами в гене <italic>MYH8</italic>.</p><p>Специфические клинические признаки дистального артрогрипоза 7-го типа в сочетании с наличием мажорного нуклеотидного варианта позволяют оптимизировать процесс молекулярно-генетической диагностики этого типа наследственного артрогрипоза.</p></trans-abstract><kwd-group xml:lang="en"><kwd>trismus-pseudocamptodactyly syndrome</kwd><kwd>distal arthrogryposis type 7</kwd><kwd>exome sequencing</kwd><kwd><italic>MYH8</italic> gene</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>синдром тризма–псевдокамптодактилии</kwd><kwd>дистальный артрогрипоз 7-го типа</kwd><kwd>секвенирование экзома</kwd><kwd>ген <italic>MYH8</italic></kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Bamshad M., Van Heest A.E., Pleasure D.
