Paired-pulse transcranial magnetic stimulation allows assessing intracortical inhibition. However, a high variability of motor evoked potential (MEP) amplitude is a limitation of its use. Therefore, a new threshold tracking technique became of particular interest, which is based on the measurement not of the amplitude, but of the change of test stimulus intensity required to induce a MEP of a given amplitude.<bold>Aim</bold>. The assessment of absolute and relative reliability of short-interval intracortical inhibition (SICI) using threshold tracking technique in healthy volunteers.<bold>Materials and methods</bold>. All healthy volunteers included into the study (n = 12) underwent diagnostic paired-pulse transcranial magnetic stimulation in two consecutive days. The procedure included registration of passive motor threshold; the registration of 30 MEPs with supra-threshold intensity and determination of SICI using threshold tracking technique. At the first day the procedure was performed twice (T1, T2), at the second day – once (T3). Standard error of the measurement (SEM) and SEM% were calculated to assess absolute reliability, and intra-class correlation coefficient – for the assessment of relative reliability.<bold>Results</bold>. A good or excellent relative reliability were observed for SICI averaged at intervals 1.0–3.0 ms and 1.0–7.0 ms when assessed within a day or at different days. Relative reliability of SICI at separate interstimulus intervals varied in a wide range. SEM% was more than 10 % both for averaged SICI and SICI at all interstimulus intervals. Motor threshold had excellent reliability both assessed within a day and at different days, and low SEM values (5.6 % for Т1–Т2 and 4.39 % for Т1–Т3). High SEM and SEM% were observed for average MEP amplitude, it also had a moderate relative reliability when assessed within a day and a poor one – at different days.<bold>Conclusion</bold>. Calculation of averaged SICI can be recommended in further studies because of its high reliability values.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение</bold>. Транскраниальная магнитная стимуляция парными стимулами позволяет оценивать внутрикорковое торможение. Применение данной методики ограничено высокой вариабельностью амплитуды вызванных моторных ответов (ВМО). Большой интерес представляет методика «отслеживания порога» (threshold tracking), при которой определяется не изменение амплитуды, а требуемое изменение интенсивности тестового стимула для получения ВМО заданной амплитуды.<bold>Цель исследования</bold> – оценка абсолютной и относительной надежности определения короткоинтервального внутрикоркового торможения (short-interval intracortical inhibition, SICI) с применением методики «отслеживания порога» на выборке здоровых добровольцев.<bold>Материалы и методы</bold>. Каждому включенному в исследование здоровому добровольцу (n = 12) в 2 последовательных дня проводилась диагностическая транскраниальная магнитная стимуляция парными стимулами, включающая определение пассивного моторного порога, регистрацию 30 ВМО при надпороговой стимуляции и определение SICI с применением «отслеживания порога». В 1-й день обследование по данному плану проводилось дважды (T1, T2), во 2-й день – однократно (T3). Для оценки абсолютной надежности (reliability) определяли стандартную ошибку измерения (standard error of the measurement, SEM) и SEM%, для оценки относительной надежности – коэффициент внутриклассовой корреляции.<bold>Результаты</bold>. Выявлена хорошая или отличная относительная надежность усредненного значения SICI для интервалов 1,0–3,0 и 1,0–7,0 мс при оценке в один день и в разные дни. Относительная надежность оценки SICI для отдельных межстимульных интервалов варьировала в широких пределах. Показатель SEM% был &gt;10 % как для усред-ненного значения SICI, так и для SICI при всех межстимульных интервалах. Определение моторного порога характеризовалось отличной надежностью при оценке в один день и в разные дни, а также низкими значениями SEM (5,6 % для Т1–Т2 и 4,39 % для Т1–Т3). Для средней амплитуды ВМО выявлены высокие показатели SEM и SEM%, средняя относительная надежность при оценке в один день и плохая – при оценке в разные дни.<bold>Выводы</bold>.
